BREAST IMPLANT POSITIONING – BEHIND THE PECTORAL MUSCLE OR IN FRONT OF THE PECTORAL MUSCLE

Twenty years ago, most breast implants were placed in front of the muscle and behind the breast gland (sub-glandular position).

Today, many plastic surgeons prefer to place breast implants behind the pectoral muscle (sub-muscular position) because we now know that:

1.	Sub-muscular breast implants have a much lower likelihood of becoming too hard (capsular contracture).
2.	Sub-muscular breast implants make mammograms easier to perform and more reliable to read than sub-glandular breast implants because more breast tissue is visible
3.	Sub-muscular breast augmentations do not allow rippling of the breast implant to become visible, except in the lower, outer corner of the breast which is concealed in a bra or swim suit
4.	Sub-muscular breast implants, when done well, have a smooth, tapering upper pole, so that the breast has a more natural appearance and the “fake looking” or “half a grapefruit” look is avoided.

Dr. Davidson occasionally places silicone gel breast implants in a sub-glandular position for body builders where intense, frequent muscle activity could move the breast implants out of their ideal position or transiently change the breast implant shape.

For more than 99% of his patients, Dr. Davidson favors the sub-muscular breast implant position for breast augmentation.
